To begin the registration process, complete this "Application for Admission" form and submit it to our director. This allows us keep to track of the number of applicants so we can determine availability for our programs. You may return the completed form to our director via email attachment ([email protected]) or by printing it and dropping it off at the church office during regular office hours (Monday-Friday, 8:00am-Noon).
